Death Certificate Requests
Overview
Only surviving spouse, children, parents and siblings of the deceased are entitled to copies of death certificates.
If a parent is divorced or was never married to the deceased, he/she can obtain the death certificate on behalf of the child of the deceased if the child is under 18 years of age. You must provide a copy of the child’s birth certificate. The birth certificate should reflect your name as well as the name of the deceased parent in order for you to obtain a copy of the death certificate.
Death Certificate requests in person
The person requesting a death certificate must complete a death certificate application and have one of the following current official identification:
•U.S. Driver's License
•Non-driver’s photo ID (issued by the Department of Motor Vehicles)
•Passport
•Additional proof of relationship to deceased is required for children and siblings:
◦For children: long form birth certificates showing deceased parents’ name(s)
◦For siblings: long form birth certificates showing their parents’ name(s)
Death Certificate requests by mail
The person requesting a death certificate may either send a completed death certificate application or a written letter with the following information included:
•Name of the deceased
•Date of death
•Your Signature must be notarized
•Photo Identification: U.S. Driver’s License, DMV Non- Driver’s photo ID or Passport
•For children and siblings, see additional proof of relationship requirements above
•A stamped, self-addressed envelope
Death Certificate by Attorneys
Attorneys representing an estate of a deceased individual may obtain copies upon providing a written letter on the law firm letterhead stating the reason for the request. Enclose your letter, the death certificate application, fee and a self-addressed, stamped envelope if by mail.
